Al instante, inmediatamente. Nosotros diríamos "al toque". A tourist came and asked to have his car shipped to Montevideo on the nail: Vino un turista y pidió que el embarcaran su auto a Montevideo al instante.
Dictionary.com nos dice que es una expresión informal pero agrega otro significado aparte del anterior:
a. of present interest; under discussion
b. without delay; on the spot; at once: He was offered a job on the nail.
El diccionario Cambridge de idioms nos trae lo mismo:
If you pay an amount of money on the nail, you pay all of it immediately. "He always paid cash, on the nail."
